Key Responsibilities

  • Conduct client assessments and develop person-centred care/support plans.
  • Coordinate services, monitor outcomes, and review care plans regularly.
  • Advocate for clients, ensuring their cultural, linguistic, and health needs are respected.
  • Support clients to access appropriate services, programs, and resources.
  • Liaise with families, support workers, and health professionals.
  • Ensure compliance with Aged Care Quality Standards.
  • Enrol and manage Home Care Packages for aged care clients.

What We're Looking For

  • Qualifications: Diploma in Community Services, Welfare, Aged Care, or related field.
  • Experience in aged care, disability, or community health settings.
  • Strong communication, problem-solving, and organisational skills.
  • Must speak Vietnamese/English (Cantonese or Mandarin a bonus).
  • Empathy, cultural awareness, and a genuine passion for supporting older people.
  • Current First Aid & CPR Certificate.
  • National Police Check (or willingness to obtain).
  • Current Driver's Licence and reliable vehicle.
  • Full working rights in Australia.
  • Ability to contribute to business growth by engaging new clients.
