Primary objective
Provide leadership and efficient co-ordination skills towards overall requirements of services (internal and external) for complex care
patients at the Alice Springs Hospital, working in conjunction with all stakeholders to ensure timely and successful patient discharge,
transition of care and prevention of readmission.
Key duties and responsibilities
1. Apply specialist knowledge and competent case management skills unique to each complex care patient to overall collate specific
assessment information related to families/carers, groups and communities to then effectively engage with professional agencies (i.e.
NDIS, local Aged Care services) and providers prior to discharge to ensure a complete and successful discharge from hospital.
- Communicates, collaborates and provides expertise relative to their discipline and clinical area and effectively engages with other
disciplines (Medical, Nursing, Allied Health) to ensure cohesive and multi-disciplinary care towards complex care patients and their
unique needs.
- Initiate, implement, monitor, and evaluate continuous quality improvement and service development activities at a local or regional
level to ensure delivery of contemporary services and pathways for complex care patients, including strategies to prevent readmission
rates.
- Demonstrate NTH values - safe; responsive; kind – in all decision-making and interactions with consumers and colleagues.
- Follow defined service quality standards, work health and safety policies and procedures relating to the work being undertaken to
ensure high quality, safe services and workplaces.
6. Foster a positive workplace culture through exemplary leadership practices and role modelling behaviour.
Selection criteria
Essential
1. Current AHPRA registration (if required for profession) or registered with nationally recognised professional body.
- Demonstrated highly developed clinical knowledge and skills in relevant area/ profession; supporting well developed clinical decision-
making and problem solving to achieve best practice outcomes for healthcare consumers.
3. Experience in creating and promoting a learning environment through clinical support, instruction, and feedback including mentoring
students, graduates and less experienced staff across the multidisciplinary team.
4. Experience and competency in leading culturally safe care, strong patient advocacy and proven experience in working effectively with
patients and families/ carers of diverse cultural and social backgrounds to ensure best health outcomes.
- Knowledge of high-level organisational structures and experience in system wide thinking that contributes to the development and
implementation of local/regional policy, including interpreting established professional precedents and organisational policies and
supporting change management.
- Proven high level communication and interpersonal skills, including well developed negotiation and conflict management skills, with
significant experience working in a multidisciplinary team environment to facilitate team functioning.
Desirable
1. Post-graduate qualifications in clinical education, leadership / management, speciality practice area or another relevant field.
