The role:

A full time opportunity for a Registered Nurse Nurse to join our team at James Milson Village North Sydney as the Health and Wellbeing Coordinator for Independent Living/Retirement Living.

This is the perfect opportunity for someone with clinical experience who is passionate about supporting older people to maintain their independence, stay well, and live meaningful lives in a community setting.

As the Health and Wellbeing Coordinator, you will lead, coordinate and deliver the operations of clinical, home support and personal care services and assist in the delivery of health and wellness programs for Independent Living Units (ILU) clients.

Duties and responsibilities include:

  • Undertake assessments for new and existing Independent Living Unit (ILU) residents and provide clinical support where required, including liaison with GPs and other health professionals.
  • Lead care staff to deliver in-home services.
  • Schedule and supervise care and hospitality staff to ensure effective service delivery.
  • Assist with wellness initiatives, group activities, and social events that strengthen connection and support healthy ageing.
  • Keep up-to-date clinical and personal records, prepare monthly service and billing reports, and provide regular updates to management.
  • Communicate with ILU residents, families, hospitals, and other service providers to support care transitions, manage wellbeing concerns, and coordinate emergency responses when needed.

Who we are looking for:

  • Currently registered with AHPRA as a registered nurse.
  • A team player, professional and self-motivated
  • Experience in the provision and coordination of care services to elderly clients
  • Highly developed interpersonal and communication skills
  • Strong customer service/relations skills and a commitment to providing quality services and implementing continuous improvement
  • Commitment to professional and self-development activities
