About the role
Part time hours available
Fixed Term - to December 2026
The Prison Transition Worker will work towards expanding QuIHNs Hepatitis C Prison Transition services with a focus on Aboriginal Torres Strait Island people who need Hepatitis C support and education in prison, in community post release, and/or at risk of entering correctional services in South-East Queensland.
- Implementing a service model for First Nations prison transitional services including managing centralised referrals, providing linkage and support to clients
- Developing and delivering culturally appropriate harm reduction education sessions for Aboriginal & Torres Strait Islander people via existing channels
- Maintaining strong stakeholder relationships both internally and externally
What we're looking for
Someone who is passionate about their work and the health outcomes of both clients and the broader community and can demonstrate:
- Tertiary qualification i.e. Aboriginal Health Practitioner, Cert IV or above, or equivalent in a relevant field
- A demonstrated understanding of the issues affecting Aboriginal and/or Torres Strait Islander people
- An ability to communicate sensitively and effectively with Aboriginal and/or Torres Strait Islander people and to engage clients who are engaged in corrective services
- Previous experience in a similar role i.e., Case Management, transitional services
- Ability to achieve a successful Criminal History Check + Admin Form 27A (Application to Visit) as required by Queensland Corrective Services
- Proficient computer skills in Microsoft Office Suite, and CMS (QFiles and Qudos)
- Current Queensland driver's licence (or equivalent)
