Mental Health Care Navigator - Medicare Mental Health Kids Hub
Service Delivery Directorate - Child Youth & Family Division
About the role
As a Mental Health Care Navigator at Medicare Mental Health Kids Hub, you'll play a core role in initiating the delivery of high-quality clinical services to children (0-12 years) experiencing mild to moderate mental health and well-being challenges. You'll take a child centred approach but will extend your scope to focus on the families and carers of our clients to contribute a truly wholistic health service in our community.
As the first point of contact with our clients you'll use your assessment and flexible problem-solving skills to the fore, to determine the needs of our clients, and shape our response to it. You'll be pivotal in engaging and supporting children and families within the Hub and guiding their support with other service providers.
You'll be working closely with a range of internal and external care providers and partner organisations across health, education, social services, Aboriginal health, and disability sectors to ensure continuity of care for our clients and increase access to support options for children in our community, which will make full use of your communication and advocacy skills.
You'll have the benefit of practicing within an experienced multidisciplinary team including allied health, nursing and medical staff reflecting the Hub based nature of the service, which will add to the scope of the support you'll receive and be able to provide.
This is a core position within the service and the team and you'll have access to ongoing support and mentoring in your role as well as opportunities for professional development.
This is a temporarily funded position until 30 June 2026 with the possibility of extension subject to funding.
