About the Opportunity
As HomePlace's Transition Coordinator, you will facilitate smooth, person‑centred transitions for people living with disability who seek to have their disability support from HomePlace. This includes coordinating all aspects of the transition process, managing enquiries, liaising with stakeholders, and ensuring each participant's transition is respectful, well‑planned, aligned with their NDIS plan and goals, and is a positive experience for them and their support network.
Your roles and responsibilities will include (but are not limited to):
- Management of new service enquiries in line with HomePlace guidelines
- Coordination of supports to be provided by HomePlace, including housing setup (where applicable) and support planning
- Liaise with families, guardians, plan nominees, support coordinators and providers to ensure continuity of support
- Facilitate pre‑transition meetings, home visits, and orientation meetings as required
- Ensure documentation, service agreements, and onboarding materials are completed
- Ensure services quoted and scheduled are delivered within each person's allocated funding for the duration of their plan.
- Ensure that individuals are effectively and authentically involved in decisions and the planning of their HomePlace supports.
- Ensure that services and supports are tailored to meet the goals of individuals and these are reflected in individual Service Agreements, rosters of care, schedules of supports, , and associated staff work plans and patterns of work.
- Ensure all documentation is current, relevant and in line with NDIS practice standards
- Coordinate the orientation and ongoing supervision and support of Senior Support Workers and/or Support Workers assigned to support each person transitioning to have HomePlace supports
- Ensure the preparation of support staff to deliver a personalised, relevant, purposeful, quality support service to people supported by HomePlace.
Ensure the principles of Person-Centred Active Support are embedded throughout the transition process
About You
With a minimum of three years' experience in disability or community services, and direct expertise in supporting people with disability and transitional arrangements., you will bring strong skills in people engagement, support planning, management of accommodation services, supported independent living, and community participation supports, complemented by experience in staff supervision, coaching and training
To be successful in this role you will need:
- Minimum 3 years in disability or community services
- Direct experience supporting people with disability
- Experience supporting transitions between providers or from family homes into shared living
- Ability to multitask and meet deadlines under pressure
- Strong experience with Microsoft Excel
- Experience working for an accommodation service or organisation providing supported independent living supports.
- Experience in leading, supervising staff and conducting staff appraisals.
- Experience in the use of computer technology, in particular the Microsoft Office suite of programs.
- Experience in supporting, coaching and training support staff.
- Demonstrated experience in undertaking project research on designated issues and prepare subsequent reports and recommendations.
- Experience in positive behaviour support
- Current Senior First Aid Certificate and CPR Certificate.
- Current NDIS screening clearance.
- Be responsive to calls from HomePlace Emergency Response line
- Be willing to provide direct support when needed and/or appropriate.
- Minimum of 3 x TGA approved and recognised COVID 19 Vaccination doses
- NDIS online modules – Quality, Safety and You and New Worker Orientation
- Current driver's licence and reliable vehicle
A degree, or equivalent in Disability Studies, Human Services or Health sector or have extensive relevant experience is essential
