About the Role
We are currently seeking an experienced and compassionate Support at Home Case Manager to join our dedicated team. In this role, you will be responsible for delivering high-quality, person-centred case management services to older clients receiving care under the Support at Home Program.

Key Responsibilities

  • Undertake comprehensive assessments to develop individualised care plans that reflect each client's goals, capabilities, and preferences

  • Coordinate and monitor the delivery of in-home and community-based services

  • Conduct regular reviews and adjustments of care plans to ensure they remain effective and relevant

  • Build strong, respectful relationships with clients, families, and service providers

  • Advocate for clients' needs and support their independence and overall wellbeing

  • Prepare, manage, and monitor individual client budgets in line with program guidelines and client needs

  • Maintain accurate and timely documentation and ensure compliance with relevant aged care legislation, policies, and standards

  • Use AlayaCare software to manage client records, care plans, and service delivery

  • Work collaboratively with a multidisciplinary team to ensure positive outcomes for clients

About You

  • Minimum 2 years' experience in case management or care coordination within the aged care, community, or health sector

  • Strong understanding of the Australian Aged Care system, including the Support at Home and/or Home Care Package programs

  • Demonstrated experience in preparing and managing client budgets

  • Familiarity with using AlayaCare or similar client management systems

  • Excellent communication and interpersonal skills, with a client-centred approach

  • Ability to work independently, manage competing priorities, and maintain attention to detail

  • Competent in using digital systems and software

  • Current driver's licence and access to a reliable vehicle

  • Current National Police Check and other relevant clearances (or willingness to obtain)

  • Tertiary qualifications in Social Work, Nursing, Allied Health, Community Services, or a related field (highly regarded)
