What You'll Be Doing
As an Aboriginal Liaison Officer, you will provide culturally safe, person-centred support to Aboriginal and/or Torres Strait Islander patients, their families and carers while they receive care at hospital. This important role builds trust in the health system and strengthens the community's perspective on what WSLHD can offer.
You Will
- Advocate for patients and families to ensure their needs are met during their hospital stay.
- Engage the community to strengthen the Aboriginal Liaison Officer process and outcomes.
- Work closely alongside social workers to achieve shared goals for patients and their families, always ensuring consent and respect are at the centre of care.
- Build and strengthen relationships between the hospital, patients, and the local Aboriginal community.
- Support discharge planning, referrals, and continuity of care through programs such as the Aboriginal Chronic Disease Management Program.
- Collaborate with hospital staff and social workers to improve outcomes for Aboriginal patients through strong engagement and shared decision-making.
- Assist with education, advocacy, and initiatives that meet National Safety and Quality Health Service Standards (Aboriginal-specific actions).

Essential Criteria
- Qualification: Holds or aspires to hold a minimum Certificate Ill qualification in Aboriginal Primary Health Care or a minimum Certificate Ill health qualification in the area of care in which the Aboriginal Health Worker works.
- Evidence of Diversity claim if applicable: Aboriginal and/or Torres Strait Islander.
Selection Criteria
- Ability to assess the needs of Aboriginal patients and to advocate on their behalf in the health system.
- Ability to establish and maintain effective working relationships with a range of health professionals across the area in working with Aboriginal patients their families and communities.
- Knowledge of a range of community-based support services and programs to assist Aboriginal patients and their families in addressing their health care needs.
- Effective interpersonal skills with a demonstrated ability to develop and maintain relationships with clients.
- Understanding of the management of patient complaints and patient privacy and escalation processes.
- Competence in the use of databases, electronic documentation, and computer based administrative systems.
- Demonstrated skill in the analysis of complex issues, problem solving, formulating new approaches and implementing comprehensive change programs and improvement strategies, including the ability to develop and monitor outcomes.
- License: Class C Driver's License.
