The heart of the role
As Care Manager you will work as part of a team of empathetic and caring clinical professionals. You will support the General Manager in managing and leading the delivery of person-centred care of residents, ensuring the highest standard of ongoing assessment, care planning and evaluation and clinical governance, helping them to live longer, healthier, and happier lives.
Duties include
- Oversee the daily clinical operations of the care home
- Support and lead the care home clinical team including coaching, mentoring, staff allocations and delegation, performance management and education
- Review clinical incidents, complaints and changes
- Lead compliance with AN ACC assessments
- Manage person-centred care planning and evaluation for residents - including case conferencing with families and carers
- Prepare and conduct clinical governance audits
- Oversee medication management in accordance with resident needs and legislative requirements
- Liaise with allied health, medical practitioners and other services to provide optimal resident care

What will you bring?
- Bachelor of Nursing with current APHRA is essential
- Post grad qualification in aged care highly regarded
- Experience managing a minimum of 30 beds
- Proven track record in delivering quality clinical care within an aged care or acute setting
- Excellent clinical assessment skills and care planning capabilities
- Highly effective communication, leadership, and relationship management skills
- An understanding of Resident Rights, Aged Care Accreditation Standards and Outcomes
