The Role

The Senior Case Manager (with a female focus) is required to:

  • Provide coordinated case management for identified clients
  • Promote early detection and intervention for mental health, effects of alcohol, other drugs and/or SEWB related issues that affect the wellbeing of the individual and/or family
  • Deliver culturally appropriate health and SEWB education programs to the Aboriginal community
  • Work in partnership with other health care professionals to ensure a multi-disciplinary approach to health care
  • Maintain information systems such as computer systems, Communicare Patient Information Recall System, data entry, statistics and other records required for the efficient and effective management of clients
  • Participate in quality assurance programs to ensure safe and effective care provision

The position is a full time permanent role.

Essential Qualifications, Skills and Experience

Aboriginality is a genuine special measure requirement of this position with the exemptions claimed under Section 65s of the SA Equal Opportunity Act 1983.

  • A Certificate IV or above in a relevant health-related area – Aboriginal and/or Torres Strait Islander Primary Health Care/Mental Health/Community Services
  • Competencies necessary to practise in the particular practice setting
  • Knowledge of Social & Emotional Wellbeing and Mental Health principles and initiatives
  • Knowledge of client-centred, consultative and community development approaches
  • Knowledge, awareness and understanding of the diversity of Aboriginal cultures and societies
  • Knowledge of Primary Health Care principles
  • Knowledge and understanding of Cultural Safety principles and their workplace implementation
  • Knowledge of Social Justice principles and practice
  • Knowledge of relevant legislation in Social Services provision
  • Knowledge of Equal Opportunity and Work Health & Safety (WHS) Legislation
  • Knowledge of relevant local resources and organisations
  • Knowledge of community needs and kinship groups
  • Experience in effective case management
  • Experience in working with female Aboriginal people
  • Experience in working in regional communities
