ABOUT US

Gateway Health is a community health service providing health and welfare services to individuals, families and communities across Northeast Victoria and parts of Southern New South Wales. We are a not-for-profit Company limited by guarantee and a registered Community Health Service under the Health Services Act 1988 (Vic).

The organisation employs over 380 staff providing a range of primary health and community services across the Ovens Murray region in Northeast Victoria and parts of Southern New South Wales. We do this by providing health care and support to individuals and communities in times of need, and by actively working with our partners to address the social and environmental determinants of health.

ABOUT THE ROLE

The Families NSW Support Worker role provides responsive and timely support, including information, and referral, where necessary to community members seeking support, and contributes to the support and wellbeing of children and families at risk.

You will be working with families that reside in NSW with at least one child 8 years or under, including pregnant women, with the Key groups being Aboriginal children, young people, families, and young parents with known vulnerabilities or who are experiencing a number of hardships.

This role is part of our Child and Family Health Program, which provides:

  • Case Management and support
  • Evidence based group work and education
  • Advocacy
  • Pathways to better health and wellbeing outcomes
  • Holistic family support
  • Child Safety and wellbeing support and education

ABOUT YOU

You are a compassionate and skilled practitioner with qualifications in welfare, community development, or human services (Diploma minimum). You bring experience in supporting children and families, using a strengths-based and trauma-informed approach.

To be successful in the role you will have

  • Excellent strengths-based, trauma informed case management skills to work with children and their families, including those from culturally and linguistically diverse backgrounds, and with complex needs.
  • A good knowledge of the health, welfare and education service system locally and regionally.
  • Highly developed interpersonal communication, problem-solving and negotiation skills.
  • Excellent time management and organisational skills, with proven ability to prioritise, work independently, and as part of a team

REMUNERATION

Community Health Centre (Stand Alone Services) Social and Community Service Employees Multi Enterprise Agreement 2022

Classification – SACS Employee Level 4 or 5 (depending on experience)
