As a Clinical Nurse – Sub-Acute Coordinator in Warwick, you'll support older patients through recovery and transition, working closely with a multidisciplinary team to coordinate care and guide clinical decisions. What your day will look like….
You'll oversee sub-acute care planning for older patients, balancing clinical leadership with hands-on support. From conducting assessments and liaising with specialists to mentoring staff and refining care pathways, your role is about ensuring safe, coordinated care that meets individual needs. You'll work with internal teams and external partners, contribute to quality improvement, and help shape a positive practice environment. In Warwick, your leadership supports not just the team—but the wellbeing of the whole community.
