About the role:

Hospital Outreach Post Suicidal Engagement (HOPE) supports people after a suicide attempt or those experiencing serious suicidal ideation, intent, or planning. Clients are supported for 12-weeks with assertive, coordinated care. Each client is matched with a HOPE Clinician and Psychosocial Support Worker, providing both practical assistance and evidence-based therapeutic interventions.

This is a Monday–Friday, business hours role, with potential for after-hours work as the program expands. The role is based at the Mental Health & Wellbeing Services (MH&WS) Melton Community Team.

The Outer Community team provides services to people in Melton including case management, acute community intervention, and the intensive support program.

Key responsibilities:

  • Triage referrals, coordinate care, and deliver individual and family therapeutic interventions.
  • Develop and review support plans with clients, monitoring progress and escalating care as needed.
  • Provide clinical leadership, supervision, and guidance within a multidisciplinary team.
  • Contribute to quality improvement through audits, KPI reporting, and service evaluation.
  • Support education, research, and suicide prevention initiatives.
  • Commit to professional growth and support the development of colleagues.

About you:

  • A recognised qualification in Occupational Therapy, Social Work, Psychology, or Mental Health Nursing, with current AHPRA registration (or eligibility where applicable).
  • Demonstrated experience in mental health assessment, therapeutic intervention, and working with complex presentations.
  • Strong clinical decision-making and leadership skills, including the ability to supervise and mentor staff.
  • Experience in suicide prevention and crisis intervention.
  • A commitment to service development, quality improvement, and evidence-based practice.
  • Excellent communication and collaboration skills to ensure coordinated, client-centred care.
