About the Role
Are you a passionate Support Coordinator who loves making a difference while working hard and having fun? Then Crosslinks could be your next great adventure starting January 2025.
We're expanding our Support Coordination team, and we've already got a caseload waiting for you—all you need to do is dive in and start helping those who need it most. If you're looking to kick off the new year with purpose, this role starts in January and offers the chance to be part of a supportive, energetic, and values-driven organisation.

Key Responsibilities
- Help participants understand and implement their NDIS plans.
- Connect individuals with services and community supports tailored to their needs.
- Monitor progress, troubleshoot challenges, and adjust strategies as required.
- Advocate for participants and ensure their voice is central to decision-making.
- Maintain accurate records and ensure compliance with NDIS guidelines.
About You
- Certificate IV or higher in Community Services (or related field).
- Strong understanding of the NDIS and person-centred practice.
- Excellent communication and problem-solving skills.
- Experience in disability or mental health services is highly regarded.
- Ability to manage competing priorities and work independently.
Mandatory Requirements
- National Police Clearance and 100-point identification check.
- Valid C Class WA Driver's Licence.
- NDIS workers clearance and NDIS workers orientation module.
- Working with Children's Check
