We offer comprehensive services to NDIS Participants and their families, including:

  • Plan Management
  • Support Coordination
  • Direct Support in all its forms – In-home, Community Access, Group Activities
  • Supported Independent Living, and Short to Medium Term Respite Accommodation

We currently have an exciting opportunity in our organisation for a passionate and organised Client Facilitator to bring their SPARK to our Direct Supports Team. This pivotal role is all about connection, coordination, and care. This position will require the right candidate to work over 5 days per/week roster, covering a minimum of 60 hours per/fortnight.

Key Responsibilities:

  • Act as the day-to-day interface between participants and Disability Support Workers
  • Listen to and identify participant and family needs with empathy and precision
  • Match participants with suitable support workers to foster meaningful relationships
  • Manage rosters and resources in a fast-paced, dynamic environment
  • Ensure supports are scheduled in line with participants' goals and NDIS plans
  • Deliver exceptional customer service—responsive, professional, and timely
  • Maintain high-level administrative processes and stakeholder engagement
  • Provide multidisciplinary advice and contribute to strategic outcomes
  • Participate in our on-call roster as part of your commitment to holistic care
