What You'll Be Doing
The Supportive Care Senior Social Worker provides a high level of Social Work services to patients, carers and families to ensure the provision of optimal health outcomes consistent with the districts and professional policies, procedures, standards, relevant legislation and Code of Conduct.

Candidates Will Need To Meet The Following Criteria
- A completed bachelor's degree in social work that provides eligibility for membership of the Australian Association of Social Workers with demonstrated experience as a Level 2 or equivalent.
- Demonstrated extensive clinical skills and knowledge in comprehensive bio-psychosocial assessments, counselling and therapeutic interventions, complex clinical casework/case management, advocacy and discharge planning.
- Comprehensive understanding and demonstrated social work experience in the social and emotional needs of patients living with degenerative disease and chronic health conditions
- Demonstrated experience in clinical and team leadership including proven capacity to teach and supervise Level 1/2 Allied Health Professionals, Allied Health Assistants and students.
- Demonstrated and high-level prioritisation and reasoning skills with proven ability to work autonomously under limited supervision, manage a demanding workload, apply reasoning skills and exercise independent professional judgement.
- Demonstrated ability to initiate, lead, complete and share quality improvement activities or service evaluation processes.
