This full-time ongoing position (hours negotiable) offers the opportunity to work as a cultural broker, ensuring seamless patient journeys for our community members.

About the Opportunity

Based in Ceduna, you will serve as the central contact point coordinating transition care for Aboriginal and Torres Strait Islander patients embarking on healthcare journeys. Working within our collaborative health system, you will implement the COAG Fixing the Gaps and Improving Patient Journey Program while reporting to the Executive Officer Director of Nursing.

Key Responsibilities

  • Coordinate comprehensive support, planning and monitoring throughout patient journeys from country and back home
  • Build health service capacity and resources for culturally appropriate patient preparation
  • Develop strong partnerships with discharge teams, Patient Liaison Nurses and Aboriginal Liaison Officers
  • Identify and address barriers within hospital and health systems
  • Facilitate access to culturally appropriate transition care across mainstream and Aboriginal Community Controlled sectors
  • Support chronic disease care plan development for Aboriginal and Torres Strait Islander patients
