About the Role

As a Care Officer, you'll be an essential part of our Home Support team, supporting the coordination of aged care and community services, ensuring client referrals and support plans are managed efficiently and compassionately.

This role is perfect for someone who thrives on organisation, multi-tasking, communication, and making systems run smoothly. Your work will directly impact the well-being of our clients by making sure services are timely, accurate, and person-centred.

Key Responsibilities:

  • Act as the first point of contact for the Home Support team across phone, email, and in-person channels by triaging, actioning or forwarding enquiries.
  • Process incoming referrals, assist with client onboarding, and support reassessments/referral allocation.
  • Maintain client referrals across My Aged Care, hospitals, and agencies, ensuring a timely response, accurate tracking, and professional communication.
  • Maintain accurate records and data in TurnPoint Care and My Aged Care portal.
  • Liaise and support internal teams (scheduling, finance, coordinators) to ensure quality service delivery.
  • Support service delivery and maintain data integrity through regular quality checks and system reporting, ensuring accurate client records, coding, compliance for audits, invoicing, and payroll.
  • Keep an up-to-date relevant knowledge of aged care referral processes.
What You'll Bring

Essential:

  • Certificate or Diploma (or working toward) in Aged Care, Individual Support, Community Services, Business, or a related field.
  • At least 2 years of administration or coordination experience in aged care, community, or disability services.
  • Familiarity with CHSP, COS, and the Wellness & Reablement framework.
  • Strong computer skills and confidence using MS Office and database systems.
  • Excellent communication and customer service skills.
  • Strong attention to detail, time management, and organisational skills.

Desirable:

  • Experience using TurnPoint Care and the My Aged Care Provider Portal.
  • First Aid Certificate, WA Driver's Licence, and access to own vehicle.
