We're looking for an experienced and passionate Team Leader to oversee our Home Care Package Program. In this leadership role, you'll support a dedicated team to deliver high-quality, client-centered care that helps older Australians remain independent at home.

You'll work closely with Case Managers and senior leaders to ensure services are effective, efficient, and aligned with client goals — while driving continuous improvement and growth in our Home Care Packages.

What You'll Do

  • Lead and support the Case Management team to achieve outstanding client outcomes.
  • Manage Home Care Package budgets and ensure compliance with all guidelines.
  • Build strong relationships with clients, families, and community partners.
  • Identify service improvements and contribute to program growth.
  • Support a positive team culture focused on compassion and quality care.

What You'll Bring

  • 5 + years' experience home care, ideally in case management
  • Strong understanding of the Aged Care Standards and compliance frameworks.
  • Excellent communication and leadership skills.
  • Budget and fund management experience.
  • Willingness to travel monthly and participate in on-call duties.
  • Based in Sydney or Tasmania.
