About the role

My Village Carers' is seeking a passionate and dedicated Support Coordinator to join our team in Bendigo, Victoria. As a part-time role 30 hours per week/ 4 days a week with the potential to grow into fulltime 38 hours, this position is responsible for coordinating and monitoring the delivery of high-quality support services to older adults and individuals with disabilities within our local community.

What you'll be doing

  • Develop and implement personalised support plans that address the unique needs and goals of each client
  • Coordinate and oversee the delivery of in-home care, social support, and community engagement activities
  • Collaborate with a multidisciplinary team of care providers to ensure seamless service delivery
  • Monitor client progress and adjust support plans as needed to meet changing needs
  • Advocate for clients and liaise with government agencies, healthcare providers, and other stakeholders
  • Maintain detailed records and documentation to ensure compliance with industry standards

What we're looking for

  • Relevant tertiary qualifications in aged care, disability support, or a related field (preferred)
  • Experience in support coordination
  • Strong understanding of the aged care and disability support sectors, including relevant legislation and regulations
  • Excellent communication and interpersonal skills, with the ability to build rapport with clients and their families
  • Demonstrated problem-solving skills and the ability to think creatively to meet client needs
  • Proficient in the use of relevant software and technology to support service delivery
  • A compassionate and empathetic approach, with a genuine commitment to improving the lives of older adults and individuals with disabilities
