Are you an experienced Registered Nurse looking for a meaningful career and lifestyle change?

We have an exciting opportunity to join our team and help strengthen primary care in the East Kimberley. Our services are expanding and we're seeking a skilled and motivated RN to drive clinical excellence and continuous improvements through our new Integrated Care and Allied Health Program.

This 12-month initiative, commencing in January 2026, will focus on improving integrated care, care coordination, and chronic condition management for people in Kununurra, WA. You'll play a vital role in implementing quality improvement strategies that enhance coordinated and integrated care pathways, helping to reduce health inequalities in this unique and vibrant community.

As the key coordinator in chronic condition management, you'll work alongside GPs to implement and monitor GP Chronic Condition Management Plans (including mental health plans). You'll collaborate closely with the GP clinic team, allied health providers, and internal and external key stakeholders to ensure care is well-coordinated, patients feel empowered in managing their health, and primary care delivery remains seamless and proactive.

About you

  • Registered with AHPRA as an Registered Nurse
  • Minimum 3 years' clinical experience
  • Demonstrated experience in Chronic Conditions Management
  • Exceptional communication, stakeholder engagement, and interpersonal skills
  • Passionate about strengths-based, responsive, multidisciplinary care
  • A systems thinker committed to continuous improvements and person-centred care
  • Able to commence December 2025
