What you'll be doing
As a
Care Navigator
, you'll be the warm, knowledgeable voice that helps people take their first steps into aged care with confidence. You'll connect with older Australians, families, carers, and referrers, listening closely, answering questions, and guiding them through the process with clarity and compassion.
From the first enquiry to service readiness, you'll help people understand their options, navigate aged care pathways, and feel supported every step of the way. You'll manage leads, follow up with care, and coordinate smooth handovers to our Care Partners, while being personable, responsive, and aligned with Suncare's values.
This role is offered as permanent full time or part time basis - please share your preference in your application.
Following an initial training and probation period, you'll have the flexibility to work in a hybrid work arrangement combining time at our Maroochydore office with working from home.
We're looking for someone who combines deep sector knowledge with empathy, adaptability, and a collaborative spirit.
You'll thrive in this role if you
- Have extensive experience across aged care funding types (SaH, HCP, STRC, CHSP, VHC, FFS), and understand how to navigate referral networks, assessment processes, and eligibility requirements with confidence
- Bring in-depth understanding of My Aged Care, with hands-on experience guiding customers through the full journey, from registration to understanding service options, contributions and income/asset assessments
- Are skilled in supporting vulnerable individuals to make informed decisions, using a culturally aware and trauma-informed approach
- Know how to identify trigger points for initiating contact with internal and external stakeholders and can confidently escalate high-risk situations or potential conflicts of interest e.g. unsafe care environments or mismatched funding types
- Communicate with clarity, kindness and professionalism with a genuine desire to help people feel seen, heard and supported
- Can juggle multiple tasks, prioritise leads based on risk and urgency and adapt to changing needs with a positive, solutions-focused attitude
- Are confident using CRM systems and open to learning new technologies
- Have experience working with marketing teams to report on campaign effectiveness and engagement
- Work well independently while thriving in a collaborative team environment
- Are highly skilled in record keeping, data entry, and documentation, including minute taking, report generation, and general administration
