About the role

The Nurse Navigator – Disability and NDIS is a Registered Nurse who demonstrates advanced levels in clinical skills, knowledge and translation of contemporary evidence-based care to practice. The Nurse Navigator aims to facilitate safe and efficient care for community patients who are complex by reducing service fragmentation, enhancing patient self-management through understanding of their condition and coordinating the connection to appropriate services to enhance outcomes thus smoothing the patient journey.

About you

Our ideal candidate will be someone who can demonstrate the following:

  • Advanced clinical evidence-based skill and knowledge in acute nursing care, coordination and interdisciplinary care management for NDIS community patients, evidenced by recency of practice and achieved clinical and professional competencies.
  • Leadership proficiency through the articulation of patient and professional engagement principles and personal attributes that influence a positive culture and achieve active collaboration across the wider health system.
  • Proficiency in the establishment of therapeutic relationships, incorporating health education, advocacy and culturally competent professional practices that facilitate achieving patient identified goals and optimised health experiences.
  • Applied knowledge of quality improvement frameworks and associated quality processes inclusive of the integration of nursing research, identifying experiences that evidence a professional commitment to contributing to quality improvement.

Mandatory Requirements:

  • Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16 (PDF) ( )
  • Appointment to this position requires registration with the Australian Health Practitioner Regulation Agency (Ahpra) as a Registered Nurse and a current annual practising certificate are mandatory.
