Advance Your Nursing Career in a Dynamic Support Team
Provide advanced clinical leadership and care for aged care residents across Metro South Health hospitals with Comprehensive Aged Residential Emergency Partners in Assessment Care and Treatment 's (CAREPACT) dedicated team. This role is Permanent/ Temporary Full time, Part time or Casual.
Your New Role
As a Clinical Nurse within CAREPACT, you'll deliver expert care and leadership supporting Residential Aged Care Facilities (RACFs) and hospitals across Metro South Health. You will provide clinical assessment, education, and emergency support to enhance patient outcomes and promote high-quality, person-centred care. You will:

• Deliver advanced clinical assessment and problem-solving for gerontic patients.
• Provide leadership, education, and guidance to nursing and multidisciplinary teams.
• Coordinate and support emergency care and discharge planning for RACF residents.
• Contribute to continuous improvement, research, and innovation within CAREPACT.

Skills*
You are a confident and compassionate nurse who thrives in dynamic healthcare settings. You bring advanced clinical skills, strong communication, and a passion for improving the quality of care for older adults. You will have:

• Registration with the Nursing and Midwifery Board of Australia (RN).
• Advanced clinical knowledge in gerontic or emergency nursing.
• Demonstrated leadership, critical thinking, and problem-solving skills.
• A commitment to person-centred care, education, and continuous improvement.

The Comprehensive Aged Residential Emergency Partners in Assessment Care and Treatment (CAREPACT) is a Residential Acute Support Service (RaSS) that offers a multifaceted program to work in partnership with RACFs and General Practitioners in providing a centralised contact for clinical support, resources and education with a focus on streamlining through the use of pathways for Residential Aged Care (RACF) residents.

The aim of CAREPACT is to improve quality of care by supporting patients and/or their substitute decision makers to be able choose where the right care is provided, at the right time, at the right place. CAREPACT also supports Metro South through Emergency Department (ED) and hospital substitutive care of RACF residents.
