Based within Lower Hunter Community Health Services, this position offers a rewarding opportunity to work as part of an integrated multidisciplinary team providing high-quality care across home and community settings.
Our team - including nurses, occupational therapists, and doctors - delivers coordinated, home-based care to support patients with complex health needs. This integrated model bridges hospital and community care, enhancing patient outcomes, supporting families, and ensuring continuity across the healthcare journey.
The team culture is collaborative, supportive, and patient-focused. We work closely together while also valuing independence and professional trust, ensuring our nurses can confidently deliver person-centred care in the community.
About the Role
As a Clinical Nurse Specialist Grade 2 you'll have a high level of autonomy, managing your own patient list and providing clinical care in the home environment, while collaborating with doctors and the broader multidisciplinary team (MDT) to ensure the best possible outcomes for patients and carers.
In this role, you will:
- Independently manage and prioritise your patient caseload
- Deliver complex clinical care and problem-solve in dynamic community settings
- Coordinate seamless transitions for patients within Supportive and Palliative Care services
- Advocate for patients and families, providing education, guidance, and emotional support
- Maintain accurate documentation and evidence-based care plans
- Be supported by the Nurse Unit Manager and the Nurse Manager of Lower Hunter Community Health, ensuring you have guidance and mentorship as you settle into the role.

Who You Are
We're seeking a skilled, compassionate, and motivated clinician who thrives in a community-based role that blends independence with teamwork.
You'll need:
- Current AHPRA Registration as a Registered Nurse
- Relevant post-graduate registration qualifications in Palliative Care with a minimum of three years' experience working in the clinical specialty
- Current Driver's License with willingness to travel for work purposes
- Strong clinical assessment and decision-making skills
- The ability to work autonomously and as part of a multidisciplinary team
- Excellent communication, organisation, and problem-solving abilities
- A genuine passion for palliative and supportive care
