About the Role

You will:

  • Coordinate and manage services for Support At Home and NDIS participants
  • Develop, review, and monitor person-centred care and support plans
  • Conduct in-home assessments and identify risks and opportunities for improved wellbeing
  • Build positive relationships with clients, families, and support staff
  • Work collaboratively with the clinical team and other providers to ensure high-quality outcomes
  • Ensure compliance with Aged Care and NDIS Quality Standards

About You

You are an empathetic, organised, and proactive professional who enjoys supporting people to achieve their goals. You'll bring:

  • Experience in case management, care coordination, or a related role within aged or disability services
  • Strong understanding of consumer-directed care, NDIS, and the new Support at Home program
  • Excellent communication, time management, and problem-solving skills
  • Ability to work independently and as part of a supportive multidisciplinary team
  • Current Driver's Licence, NDIS Worker Screening (or willingness to obtain)
  • Relevant qualifications in Aged Care, Community Services, Nursing, Allied Health, or Case Management
