Job Summary:

HEALTHIQ CONSULTING & SUPPORTS PTY. LTD is seeking experienced and compassionate Disability Support Workers to join our team in providing high-quality care to people with disabilities across Perth, Western Australia. The successful candidate will support clients in daily living activities, community participation, and personal care, in line with NDIS standards and company policy.

Duties and Responsibilities:
  • Provide individualised support services to clients with physical, intellectual, or psychosocial disabilities.
  • Assist clients with daily living tasks, including hygiene, dressing, and meal preparation.
  • Support clients in accessing community, social, and recreational activities.
  • Document support services and maintain accurate progress notes.
  • Follow care plans and behaviour support strategies.
  • Liaise with family members, coordinators, and allied health professionals.
  • Ensure compliance with organisational policies, NDIS Code of Conduct, and WHS standards.
Skills and Qualifications:
  • Certificate III or IV in Disability, Individual Support, or Community Services (or equivalent).
  • Minimum 1 year relevant experience.
  • Knowledge of NDIS and person-centred support models.
  • Valid driver's licence and current NDIS Worker Screening Check.
  • Excellent interpersonal and communication skills.
