Austin Health is the major provider of tertiary health services, health professional education and research across three campuses in the north-east of Melbourne and statewide. We have a highly engaged, diverse and collaborative workforce that is unified by our goal to help people live healthy and fulfilled lives.
About the Nutrition and Dietetics Department
The Nutrition and Dietetics Department is part of the Allied Health division, which employs over 1,000 health professionals across 27 disciplines, broadly known as the therapies and sciences, who work as key members of the multidisciplinary team aiming to provide the best outcomes for patients via preventing, diagnosing and treating a range of healthcare conditions.
Our department has around 55 staff members across three sites - Austin (acute care), Repat (sub-acute care) and Talbot (rehabilitation), providing services to inpatients, outpatients and home & community programs. We operate as one department and come together for staff meetings and continuing education. Our work environment is vibrant and fast paced and we are a friendly and supportive team. We have many highly skilled and experienced dietitians who lead our clinical teams - specialist medicine, oncology, surgical/ICU/gastro, paediatrics and mental health, sub-acute and ambulatory care (including home based programs) and portfolios - staff education, quality, research, student training, food service, nutrition support and workforce operations.
About the role
As the Senior Dietitian for and Subacute Home Based Programs you will work as part of the Subacute team of Dietitians and will provide clinical expertise and leadership to Dietitians working in these areas.
Your primary workload allocation will be providing specialist nutrition assessment, care planning, and intervention to older adults and patients with complex medical or psychosocial needs in the Acute and programs.
These programs include services to patients receiving home-based multidisciplinary care following a hospital presentation or admission, with the aim of avoiding unnecessary hospital stays, promoting recovery in the home environment, and reducing the risk of hospital-acquired complications.
The programs are collaborative, patient-centred, and future-focused - designed for clinicians who are passionate about delivering innovative models of care that prioritise dignity, independence, and wellbeing at home.
You will be primarily based at the Austin Hospital, with home visits required. Your workload allocation may vary according to operational needs across the stream and the department. You will be working 24 hours per week (0.63 FTE) across three or four days, with roster ideally including Friday, as well as either Tuesday or Wednesday (to attend interdisciplinary case conference).
About you
You will be an experienced and proactive clinician who brings excellent communication, clinical leadership, and a strong commitment to excellence in patient-centred care. You'll be adaptable and collaborative, with a passion for supporting patients to achieve their health and recovery goals in the home setting.
To be successful in this role you'll bring with you:
- Have a minimum of 7 years' experience as a dietitian
- Deliver comprehensive nutrition assessments and interventions tailored to patients in acute and subacute home-based programs
- Collaborate closely with multidisciplinary teams to ensure integrated and coordinated care
- Provide clinical supervision and mentorship to early-career dietitians and students
- Contribute to service development by identifying opportunities to improve access, equity, and outcomes in home-based care models
- Participate in team meetings, case conferences, and service evaluation activities
- Have a current driver licence

- A suite of wellness initiatives designed to support you, including discounted fitness memberships and special offers on health insurance, and a comprehensive wellbeing program.
