Join Us as a Nurse Practitioner or Nurse Practitioner Candidate – Geriatric Medicine – Deliver Advanced, Person-Centred Care for Older Adults
Are you ready to make a lasting impact in geriatric health? We're seeking a Nurse Practitioner or Nurse Practitioner Candidate to provide expert clinical leadership and advanced care for older adults across community, residential aged care, and hospital settings, supporting ageing in place and reducing avoidable hospitalisations.
Nursing Careers at Limestone Coast Local Health Network
Key Responsibilities:
- Deliver care within the acute setting to patients transitioning to residential aged care, preventing unnecessary hospital transfers.
- Support patients with memory loss and Behavioural and Psychological Symptoms of Dementia (BPSD).
- Collaborate to develop and implement patient-centred management plans.
- Support integration of care between LCLHN acute facilities, RACFs, and other providers.
- Provide education and training to ward/unit staff, community programs, and medical/care staff.
- Lead and coordinate projects, programs, and research to enhance patient care, education, and service delivery.
- Support education and supervision of student nurses, enrolled nurses, and early-career registered nurses.
- If appointed as a Nurse Practitioner Candidate, expand your practice through postgraduate study, mentoring, and clinical supervision to achieve endorsement.
What We Are Looking For:
- Registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia, holding (or eligible to hold) a current practising certificate.
- Nurse Practitioner: Endorsed (or eligible) by the NMBA as a Nurse Practitioner.
- Nurse Practitioner Candidate: Currently enrolled (or eligible to enrol) in an NMBA-approved master's program, with at least 3 years' post-registration experience.
- Demonstrated clinical management and leadership experience, including program evaluation and service development.
- Experience in developing, implementing, and evaluating quality improvement initiatives, policies, and procedures in community or acute settings.
- Knowledge of and commitment to the National Safety and Quality Health Care Service Standards, Aged Care Quality Standards, and NDIS Standards.

Applicant Information
This is a part time vacancy, working 37.5 hours per fortnight and salary will be adjusted according to actual hours worked.
