Mulungu is an Aboriginal Community Controlled Health Service committed to improving health and wellbeing for Aboriginal and Torres Strait Islander peoples in Mareeba and surrounding districts. Our service model integrates Health Services, Social Services, and Corporate Services to deliver comprehensive care under one umbrella, fostering strong relationships with government departments and the local community.

Position OverviewThe Care Coordinator - Integrated Team Care (ITC) plays a vital role in coordinating care for patients with chronic and complex health conditions. The successful candidate will be responsible for improving health outcomes for referred clients by effectively managing healthcare activities and ensuring seamless communication between various health service providers. This role is essential in enhancing access to culturally appropriate care for Aboriginal and Torres Strait Islander children and families.
Key Responsibilities- Operational Management: Ensure cohesive service delivery aligned with Mulungu's strategic direction.
- Client Servicing: Assist clients with managing health services, providing culturally appropriate care, and advocating on behalf of Aboriginal and Torres Strait Islander clients.
- Quality Improvement: Commit to high-quality management practices and contribute to continuous improvement initiatives.
- Leadership and People Management: Foster a collaborative workplace culture and provide guidance to the health services team.
- Resource Management: Engage in data collection and reporting to enhance service delivery and program growth.
- Networking: Maintain and develop relationships with partner organizations to benefit service delivery.
The ideal candidate will have a strong commitment to improving health outcomes for Aboriginal and Torres Strait Islander communities. They will demonstrate a combination of relevant qualifications, experience, and personal attributes that align with the mission of Mulungu.
- Qualifications: Relevant qualification as an Aboriginal Health Practitioner Certificate IV or Registered Nurse with current AHPRA registration.
- Experience: A minimum of 3 years in a similar role, with demonstrated experience in working with Aboriginal and Torres Strait Islander communities.
- Skills: Well-developed interpersonal skills and the ability to communicate effectively with diverse stakeholders.
- Knowledge: Knowledge of or ability to acquire knowledge of Pracsoft and Medical Director databases.
- Blue Card -Working with Children, Current National Police Check (within the last six months) Valid Drivers Licence
SC 1 Demonstrate relevant qualification and/or registration as an Aboriginal Health Practitioner Certificate IV or be a Registered Nurse with current AHPRA registration.
SC 2 Experience working with Aboriginal and / or Torres Strait Islander communities and demonstrated knowledge of Aboriginal and Torres Strait Islander cultures.
SC 3 Demonstrated advanced knowledge and experience in the Microsoft Office Suite of programs and basic knowledge of Pracsoft and Medical Director software with the ability to quickly acquire knowledge and work with new Information Technology systems and programs.
SC 4 Well-developed interpersonal skills with the ability to communicate effectively with people, particularly Aboriginal and Torres Strait Islander peoples.
SC 5 Demonstrated ability to provide leadership, training and co-ordination of systems and processes.
SC 6 Knowledge of and/or involvement in quality improvement processes.
