About the role

We are seeking a part time (30.4hours per week) Diabetes Nurse to join our community health team in Bacchus Marsh and Melton. The team offers individual and group based interventions across a diverse population group in the west. We work alongside a multidisciplinary team including nursing, dietitians, podiatrists, exercise physiologists and physiotherapists to name a few.

The role will work across outpatient and community based services seeing a wide variety of clients and have opportunity to work across all community based locations (Melton, Bacchus Marsh and Caroline Springs).

Key responsibilities:

• Provide individualised support to clients needing diabetes management
• Support group based service delivery for community based clients
• Support the multidisciplinary team

About you:

• You have experience in community environments is desirable
• You have good interpersonal, verbal and written communication skills
• Will have a passion for delivering high quality care working collaboratively with a multidisciplinary team
