About  

The Program facilitates the shift of traditional inpatient acute and rehabilitation services from all three Austin Campuses to the patient's own home without compromising patient care. It improves patient satisfaction, reduces the cost of hospitalisation per in-patient episode of care and helps with hospital flow.

– Acute is a new 4-6 bed program apart of the continuing care division. Our goal is to help more older people who present to the Emergency Department (ED) access the care they need in the right location—at home instead of in hospital whenever possible. We recognize that many older patients can be safely and effectively managed in their own homes with the right supports in place.

About the role 

The Clinical Nurse Consultant will lead rapid assessment, acute nursing care, care planning, and coordination of services to facilitate safe discharge from ED or other inpatient wards as determined to home-based care. This includes working closely with ED teams, community health providers, primary care, and support services to optimise patient outcomes. The role involves clinical expertise, service innovation, and collaboration to support the successful implementation and evaluation of this new model of care.

This is a fixed term parental leave cover working 32 hours per week until 30 August 2026.

A regular day will see you:   

  • Driving to visit admitted patients in their own homes in Austin Health's catchment
  • In collaboration/consultation the Acute team provide acute rapid assessment and short-term treatment of patients in their own home.
  • Provide an advanced level of clinical knowledge and expertise in the assessment and treatment of chronic and complex medical issues.
  • Triage referrals and support onboarding as required.
  • Work with medical/nursing/allied health teams to determine appropriate discharge plan to alternative community service or to their GP. ·
  • Practice within own scope of practice in accordance with legislation affecting nursing practice and healthcare.

About you 

As the Clinical Nurse Consultant, you will be someone with a high level of assessment skills, excellent communication skills, with a strong commitment to excellence in Geriatric evaluation and management and rehabilitation care.

You'll further bring with you: 

  • Nursing and Midwifery Board of Australia Registered Nurse registration.
  • Extensive acute care, aged care or Emergency nursing care experience.
  • Current driver's license.
  • Meet the selection criteria within the position description.
  • The ability to work independently and flexibly alongside the muti-disciplinary team to meet varying patient demands.
