About the Opportunity
The Allied Health Reablement Team (ARHT) is seeking an experienced Allied Health Professional (OT, PT, SW, SP or DT Level 4) to lead an established team.
ARHT is a Commonwealth Health Support Program (CHSP) funded team which services SWSLHD. This team leader would coordinate and lead team members in the North sector (LGAs of Bankstown, Liverpool & Fairfield), whilst working collaboratively with the team leader in the South sector (LGAs of Macarthur & Wingecarribee) to cover the needs for the District.
ARHT has occupational therapy, physiotherapy, social work, dietitian, and speech pathology services available for aged care clients eligible to access CHSP funded services through My Aged Care.
In this position you will be tasked with ensuring an efficiently run service, maximising client outputs, generating data and reports as required, and will be operationally responsible for the multidisciplinary team on a day-to-day basis.
You will have a strong understanding of the aged care system, demonstrated experience in leadership, an ability to work with competing demands and a desire to see high quality allied health care being provided to clients accessing the service.
This position will be based at Allied Health Reablement Team - Camden Site (next to the Camden Hospital).
What You'll be Doing
Coordination of a Community Home Support Program (CHSP) funded multidisciplinary allied health team providing high-quality support, at a low intensity on a short-term episodic basis, or higher intensity services delivered on a short-term basis, to frail older people to maximise their independence at home and in the community.
The team is one of the community teams managed by Community Aged Care (CAC) Services of the SWSLHD.
The position is responsible for managing admission, service provision and discharge planning for care recipients within program requirements as well as management of a multidisciplinary team of Allied Health clinicians (Physiotherapy, Occupational Therapy, Social Work, Dietetics, Speech Pathology).
The position will also carry a small clinical caseload.

SWSLHD Primary & Community Health provides a comprehensive range of community-based services designed to improve and maintain the health and wellbeing of individuals and communities across South Western Sydney.
These services include prevention, early intervention, assessment, acute and post-acute treatment, health maintenance, and continuing care. The team supports diverse populations, offering specialised programs for children, young people, older adults, and at-risk communities.
With over 800 dedicated staff, including Aboriginal Health Workers, nurses, doctors, and allied health professionals, the service ensures high-quality care close to where people live.

- Recognised degree in Allied health with current authority to practice as an Allied Health clinician (OT, PT, SP, SW, Dietitian) with Australian Health Practitioners Regulation Agency (AHPRA) and where registration not required is eligible for membership with relevant professional association.

- Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
