We're seeking a compassionate and detail-oriented Home Support Assessor to conduct face-to-face assessments for clients referred via the My Aged Care Gateway. Using the National Screening and Assessment Form (NSAF), you'll work closely with clients to identify their goals, develop tailored support plans, and connect them with appropriate aged care and community services. This role involves preparing for assessments, verifying client information, conducting screenings, and updating records in line with RAS Guidelines and Aged Care Quality Standards. You'll deliver person-centred care using wellness and reablement approaches, ensuring timely responses to referrals and ongoing review of support plans to meet evolving client needs.

What you will bring to the role
- Certificate III in Home Care or Aged Care or equivalent work experience.
- Statement of Attainment 2 and Self -Paced Learning Experiences x 3 (SPLEs) or willingness to undertake when commencing the role.
- Well-developed written, verbal and interpersonal communication skills including accurate spelling, good verbal skills and comprehension and ability to deal courteously with people at all levels.
- Demonstrated computer skills with capacity to learn and use electronic reporting systems and registration system.
- Demonstrated ability to work independently as well as effectively in a collaborative team environment.
- Ability to follow guidelines relevant to position as well as demonstrated problem solving capacity.
- Current Driver's Licence and willingness to use for work purpose travel.
Employment of a temporary visa holder may only occur if no suitable permanent resident or citizen of Australia has been identified and must not exceed the duration or conditions associated with the current visa.
