ABOUT THE ROLE

We're seeking a compassionate and proactive Case Manager to join our Bendigo team. Reporting to the Case Manager Team Leader, this role is being offered as part-time (0.6 FTE / 3 days per week).

This is a highly mobile role, you'll spend much of your time on the road visiting clients in their homes or working from home when managing care plans and administration. It's perfect for a nurse who values flexibility, autonomy, and the chance to make a real difference in the community.

You'll deliver person-centred care coordination to a diverse client group, including government-funded and private clients. This includes conducting assessments, developing care plans, monitoring wellbeing, and ensuring services stay within budget while aligning with each client's goals.

AS A CASE MANAGER, YOU WILL:

  • Visit clients in their homes to complete intake assessments and consultations.
  • Develop, implement, and regularly review individualised, goal-based care plans.
  • Coordinate services and ensure delivery remains within client budgets and program guidelines.
  • Oversee client expenditure for Home Care Packages and ensure compliance with funding rules.
  • Build strong professional relationships with clients, families, and care teams.
  • Monitor client wellbeing, respond to changes in needs, and manage risks.
  • Maintain audit-ready client files and ensure compliance with government and internal processes.

ABOUT YOU

You're a caring professional who thrives in the community, building meaningful relationships while ensuring clients receive high-quality, well-coordinated care. You'll also be confident working autonomously, with excellent organisation and communication skills.

Ideally, you'll bring:

  • Registered Nurse (RN) or Enrolled Nurse (EEN) qualifications (preferred).
  • Experience in community care, home-based care, or case management.
  • Strong customer service and communication skills.
  • The ability to manage budgets and resolve problems with a solutions-focused mindset.
  • A proactive, continuous-improvement approach.
