Position Title: Practice Nurse (RN)

Employment Type: Part-Time (3 days/week, with flexibility to cover staff leave as needed), starting November 2025

Location: Serpentine Medical Centre, Serpentine WA 6125

Position Overview:

We are seeking a compassionate, professional Registered Nurse to join our team, working three days per week with the flexibility to provide additional cover during periods of staff leave. The ideal candidate will have excellent clinical skills, a commitment to patient-centered care, and the ability to work independently as well as part of a multidisciplinary team.

Key Responsibilities:

  • Provide clinical nursing services within the scope of a general practice setting, including:
  • Immunisations and vaccinations
  • Chronic disease management (e.g., diabetes, asthma care plans)
  • Wound care management
  • Health assessments (e.g., 45–49-year-old checks, over-75s assessments)
  • Assisting with minor procedures
  • Patient triage and urgent care support
  • Maintain accurate and up-to-date clinical records
  • Educate patients on preventative health strategies
  • Assist in infection control and cold chain management
  • Participate in quality improvement activities
  • Support the coordination of recalls and reminders
  • Help on reception as needed

Essential Criteria:

  • Current Registration with AHPRA as a Registered Nurse
  • Minimum 1–2 years' experience in a primary health care or general practice setting (preferred)
  • Current CPR/First Aid certification
  • Immunisation certification (or willingness to obtain)
  • Excellent communication and interpersonal skills
  • Strong organisational and time management abilities
  • Ability to work flexibly, including covering additional shifts when colleagues are on leave
  • Proficient in the use of clinical software (e.g., Best Practice, Medical Director, or similar — desirable)

Desirable:

  • Experience with Medicare chronic disease management item numbers
  • Previous experience with quality accreditation processes
  • Previous General Practice experience
  • Previous chronic disease management experience
