We are currently seeking a passionate Lead Case Coordinator to join the Family Wellbeing team full-time

Yulu-Burri-Ba Aboriginal Corporation for Community Health (Yulu-Burri-Ba) provides a safe, friendly, confidential, and culturally appropriate Aboriginal and Torres Strait Islander Primary and Social Wellbeing Health programs in line with community needs. It is our vision to empower our Aboriginal and Torres Strait Islander families and community to achieve excellence in their healthcare outcomes.  Yulu-Burri-Ba has clinics at Capalaba, Dunwich on North Stradbroke Island and Wynnum.

The team is embedded within a Primary Health Care Clinic that provides a comprehensive range of primary and specialist care services, including but not limited to GP, Allied Health, Psychiatry, Family Wellbeing, Paediatric services, Birthing in our Community, Deadly Choices and Kinship & Foster Care Services. This role works actively with these other disciplines to ensure that the needs of clients are met in a truly holistic way.

*Please note: To be appointed to this position, you must have:

  • Current Blue Card Working with Children Check (Paid Blue Card).
  • Current "C" Class Driver's Licence Valid in Queensland.
  • Proof of immunisation record (Immunisation History Statement) – or willingness to obtain as a matter of priority.

About the Position

As a key member of the Yulu-Burri-Ba Family Wellbeing team, the Lead Case Coordinator will effectively engage with Aboriginal and Torres Strait Islander families to support services and conduct initial comprehensive assessments, utilising narrative therapy and family-led approaches to identify the real and genuine needs of a family.

The position advocates for families to ensure they are treated with respect, dignity, empathy; receive the best services; and can express their autonomy to be actively involved in all decisions that affect their lives.

General Responsibilities

  • Identify and work with extended family to build family solidarity and ongoing social support.
  • Deliver targeted supports to a wide range of clients and presentations, as per the case plan.
  • Appropriately respond to critical incidents, providing wellbeing first aid to patients in distress.
  • Provide high quality case management practices.
  • Coordinate all services to the family, as part of the Singe Family Care Plan, led by the needs and aspirations of families.
  • Liaise with other community organisations and government agencies to provide coordinated and timely access to support services and pathways of referral into the Family Wellbeing Service.
  • Provide direct troubleshooting and care planning to overcome access barriers for families.
  • Provide crisis care support.
  • Advocate for the rights and fair treatment of all families, to be treated with respect and dignity, ensuring decisions that involve families are led by families.
  • Cultivate productive working relationships at all levels, which support the nurturing of internal and external team relationships, facilitates cooperation, trust and respect, values differences and diversity and guides/mentors all Yulu-Burri-Ba staff.
  • Developing and maintaining and reporting systems.

Selection Criteria

Essential

  • Ability to communicate sensitively and effectively with Aboriginal and Torres Strait Islander people, communities, leaders and health professionals.
  • Ability to work with a culturally and socially diverse group of clients, and apply a culturally responsive framework to guide interventions, with the support of staff and management.
  • Ability to work across all three Yulu-Burri-Ba clinics (North Stradbroke Island, Capalaba and Wynnum).
  • Demonstrated ability to apply a high level of practice judgement when working with clients in setting and achieving goals.
  • Ability to work with a culturally and socially diverse group of clients, and to apply a culturally responsive framework to guide interventions, with the support of staff and management.
  • Ability to record accurate and consistent case notes.
  • Demonstrated ability to work with a minimum of direction and to effectively communicate with and work collaboratively as part of a multi-disciplinary team, to contribute to the development of creative and innovative services.
  • A willingness to provide social work support and engagement to patients outside of a clinic environment.

Qualifications/Experience

  • Certificate IV qualification or higher in Social Work, Human Services, Behavioural Science, Psychology, Criminology, Allied Health, Occupational Therapy, Sociology, Public Health or relevant field.

Desirable but not mandatory

  • Bachelor qualification in either Social Work, Human Services, Behavioural Science, Psychology, Criminology, Allied Health, Occupational Therapy, Sociology, Public Health or relevant field.
  • Previous experience in a family support and or advocacy role.
