About the role:

At Anglicare Central Queensland, we're committed to support mental health recovery through connection, compassion and empowerment. As a not-for-profit organisation, we empower individuals living with severe and persistent mental health challenges to work towards their goals in a safe and supportive enviornment.

We're seeking a Wellbeing Facilitator to support adults through individual sessions and group activities, using strength-based, person-centred approaches to co-design holistic and tailored recovery plans. You'll collaborate with care teams and staekholders to deliver proactive, recovery-oritentated services that make a lasting impact.

What will a typical day look like?

  • Establish a positive collaborative working relationship with people experiencing severe and persistent mental health challenges, their chosen care team, and key professional stakeholders to collectively work toward an identified preferred future.
  • Provide co-designed individual and group-based recovery-oriented services that are timely, proactive, person-centred and strengths based.
  • Work alongside participants and their clinical case managers, nominated health professionals and/or partner organisations, to deliver the best possible comprehensive service and outcomes.
  • Maintain participants' records to a high standard, compliant with statutory obligations and AnglicareCQ's policies.
  • Co-design individualised care, safety and recovery plans with people including implementation and review that include goals and aspirations for recovery.
  • Develop strong partnerships with key stakeholders.
  • Encourage self-agency from service commencement to transition.
  • Completion of assessment, planning and other documentation required by AnglicareCQ and funding bodies.

Mandatory Requirements:

  • Diploma level qualifications (or higher) in a Mental Health related field.
  • Full COVID-19 Vaccination
  • Ability to obtain a Blue Card (working with children check positive notice or exemption)
  • Ability to obtain a Yellow Card (disability services positive notice or exemption notice)

Desirable:

  • 3 years relevant experience.
