The Role:

An exciting opportunity exists for an enthusiastic and experienced Social Worker, who is committed to lifelong learning, to join the team at Northern Health. The Social Work team has a positive culture where team members share knowledge and experience and help each other with referrals, to ensure our patients get a timely service.

This is an ongoing part-time position, 3 days per week. The successful candidate will be eligible for 5 weeks of Annual Leave.

The role is based working with oncology outpatients and patients of our day oncology service. The incumbent will work collaboratively with other experienced Grade 2 Social Workers on the inpatient oncology ward. The incumbent will also have opportunity to work across a range of wards (medical, surgical, maternity, pediatrics and intensive care) on a duty basis.

The Grade 2 Social Worker will provide leadership and ensure high level, evidence – based Social Work services to patients. The Worker will need to demonstrate flexibility to provide an efficient response to referrals. Social Workers work closely with Allied Heath, nursing and medical colleagues.

As a Grade 2 Social Worker there will be opportunity to develop non-clinical skills in staff and student supervision, quality improvement, contributing to working parties or committees. The Social Work team runs a social work specific internal professional development calendar. The Allied Health Education and Training Division provide regular professional development for Allied Health clinicians and staff are encouraged to access external professional development that is relevant to their role and learning goals.

To be successful in this role you will have:

  • Skills and experience in providing evidence-based, high-quality care for patients with a cancer diagnosis.

  • Provide consultation and work collaboratively with all health professionals, sharing assessments with the multi-disciplinary team

  • Have a thorough working knowledge of community services and programs

  • Demonstrated experience in prioritizing, discharge planning and independently managing complex social situations

  • Have excellent engagement and communication skills.
