**CLINICAL CARE COORDINATOR MT PLEASANT WA**

Type: Full-time, 12-month contract

Pay Rate: Up to $65 per hour plus super

Key Responsibilities:

- Coordinate and manage the delivery of clinical care services to Australian military veterans
- Develop and implement care plans based on individual needs and goals
- Provide clinical supervision to team members, including psychologists, occupational therapists, registered nurses, and social workers
- Collaborate with other healthcare professionals to provide integrated care to veterans
- Maintain accurate records and documentation of all clinical care activities

**ESSENTIAL REQUIREMENTS**
- Qualification as a registered Psychologist / Mental Health Occupational Therapist / Mental Health Registered Nurse with full AHPRA registration, 5 years experience

OR
- WWC, National Police Clearance
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Understanding of the unique needs of military veterans
