**Role Description**

This is a full-time on-site role for a Disability Team Leader to Support Workers located in Maylands, WA and other nearby houses owned by Enhance Hope Services. The Disability Team Leader will be responsible for coaching and mentoring support worker about the best practices in disability care, overseeing operations within the assigned house and managing responsibilities of team members to ensure quality care for clients.

**Key Roles**:

- Ensuring team compliance about documentations (Progress Note, Incident Reports, Medication Signing Sheets & Charting)
- Ensuring medications of clients are updated and administered based on the standard.
- Ensuring team compliance on care-plan, organizational policies and procedures.
- Maintaining client satisfaction, conducting investigations about client concerns, facilitate investigations on valid complaints.
- Ensuring team compliance on shift attendance.
- Conduct house/facility checking
- Serve as a main communication channel between staff, management and clients.
- Facilitate client onboarding, assisting clients on booking and appointment setting, assist the management team about record keeping of client, organizing community access & activities of client, assist in sourcing possible clients.

**Qualifications**
- Proven work experience in community or disability sector.
- With driving rights in Australia.
- Proficient in English language.
- Has a valid NDIS Clearance Check, First Aid & CPR certificate, Medication Competent, completed Manual Handling training
- Flexible and ready to take various challenges
- Amenable reporting to work onsite on the physical office (Maylands) & house assigned.
- With valid Certificate III or Diploma related in Individual Support, Disability or others related.
- Available working 5 days a weeks - morning shift.
- Weekend off but if necessary or operations requirement might be available working too.
- Computer literate
- Knowledgeable in Microsoft Word, Excel, Power Point Presentations and Google workspaces.

**Job Types**: Full-time, Part-time

Pay: $35.00 - $40.00 per hour

Expected hours: No less than 24 per week

Schedule:

- Monday to Friday
- Morning shift

**Education**:

- Certificate I - IV (required)

**Experience**:

- support working: 1 year (required)
- team leading / supervisory: 1 year (preferred)

Licence/Certification:

- Full Driver Licence (required)

Work Authorisation:

- Australia (required)
