**Description**:
The objective of Home Care Packages and Home Support programs is to provide an individualized service to consumers with the aim of facilitating the achievement of their goals related to independence and wellbeing. These programs support individuals to live independently in the community by providing individual tailored and goals focused packages of support.

The support worker is responsible to provide quality person centered programs, ensuring that consumers have maximum opportunity for choices, self-advocacy and community participations.

To support consumers living in their own homes in their day-to-day living and support of consumer’s rights.

**Responsibilities Include**:
Provide direct care assistance to each consumer in accordance with their care plan and as directed by the Case Manager/Coordinator/ Service Manager.

**Type of services including (but not limited to)**:
personal care (showering/bathing,
dressing/grooming,
toileting,
meal preparation,
social support,
transport (with shopping, visit doctor or attend social activities),
respite,
medication assistance

domestic assistance

Providing consumer with additional support where required, such as peg feeding where appropriate or as directed.

Assist with consumer’s physical and mobility requirements in accordance with training provided and the relevant workplace health and safety standard, including transferring of consumers.

Working in accordance with the medication policy, work health and safety, consumer’s rights policies including privacy and confidentiality.

Monitor the safety and well-being of the consumers and report any concerns to the supervisor.

Work with consumers, family/carers, relevant Case Manager/Service Officer and other professionals to support the consumer’s individual goals.

Ensure completion of appropriate documentation. Ensure that relevant information on support provided is recorded in consumer notes as needed.

All other duties as directed by your supervisor.

**Skills and Experiences**:
**Selection Criteria**:
Minimum of Certificate III in Individual Support or equivalent

Professional communication with a focus on customer service

A second language is desirable but not essential

Ability to identify and understand challenges facing people from culturally and linguistically diverse backgrounds.

Commitment to team work and an ability to work independently

Current Senior First Aid Certificate

A current and valid NSW Driver’s License

Own reliable car and car insurance
