**Position Description**

Due to the rapid growth of the business, we are looking for an experienced and motivating professional to join the team. You are responsible for the providing support, information and advice to clients on emotional, financial, recreational, health, housing and other social welfare matters, and evaluate and coordinate the services of welfare and community service agencies.

**Duties & Responsibilities**
- Provide clients with information, interview, identify and assess their needs.
- Interviewing clients and assessing the nature and extent of difficulties
- Planning, developing and implementing educational, training and support programs.
- Assist clients with community participation and find resources as needed for their health, welfare, housing, employment, training etc, liaise with community groups and welfare agencies, government bodies and private businesses as needed.
- Provide support & mentoring to other support workers to assist in the provision of care to disabled people within the community
- Monitoring and reporting on the progress of clients
- Supporting families and providing education and care for children and disabled persons in adult service units, group housing and government institutions
- Assisting to solve social, emotional and financial problems
- Preparing submissions for funding and resources, and reports to government bodies and other agencies
- Provide daily service provision to all clients as per their support plans including meals, transport, personal care, social support as well as physical activities
- Act quickly and responsibly in cases of emergency
- Perform other duties within scope of preparation and as directed

**Skills & Experiences**

To thrive in this role, it is essential you demonstrate the following experience and behaviours:

- Strong experience and knowledge in disability, mental health, and/or positive behaviour support.
- Demonstrated experience in providing complex behaviour support to client who needs a stable and dedicated team around them
- Certificate III Individual Support (Disability) or equivalent is desired
- 2 years’ work experience in disability service area or similar/related area
- Possession of a current national Police Check with no disclosable outcomes.
- Mandatory COVID-19 vaccination with a TGA approved vaccine and ability to provide proof of vaccination status before being appointed to the role.
- Possession of a valid Australian driver's license.
- Demonstrated physical capability to fulfil all job duties to our satisfaction.
- Current First Aid Certificate and CPR
- Strong team spirit and willingness to work with individuals with disabilities
- Intermediate IT skills and knowledge
