**What You Will Be Doing**
The Chronic Disease Monitoring Service Team Leader is a valued member of the Community Health Leadership Team, committed to upholding Peninsula Health’s values and meeting organisational goals.
The aim of the service is to improve chronic disease management and prevent hospital admissions. Each participant will be assessed and offered a suitable device/s, depending on their digital literacy and the severity of their condition. Each participant will be monitored remotely by the monitoring service team on a daily basis.
The Team Leader position will manage a team of Registered Nurses and Allied Health staff, with Medical support. The Team Leader will collaborate with key stakeholders to implement the new model of care considering; clinical risk, inclusion criteria, intake processes and clinical escalation processes.
- Implementation of monitoring service agreed model of care in conjunction with the Program Manager and task group
- Undertake recruitment and selection of delegated staff
- Define roles and responsibilities within the team, with clear escalation pathways developed
- Provide line management, supervision, support and coaching for monitoring clinicians
- Work in partnership with other clinicians and managers to ensure the model is integrated with other services and streamline the journey for patients
