**Key Responsibilities**:

- Provide personalized care and support to people with disabilities in accordance with their individual care plan
- Assist clients with daily living activities, such as personal hygiene, meal preparation, and household tasks
- Help clients achieve their goals by providing support, guidance, and encouragement
- Monitor clients' health and well-being, reporting any changes or concerns to the appropriate healthcare professionals
- Maintain accurate and up-to-date records of client care and progress
- Collaborate with other healthcare professionals, such as doctors, nurses, and therapists, to provide coordinated care to clients
- Attend training and professional development activities to enhance skills and knowledge
- Adhere to all policies, procedures, and regulations related to disability support services
- Work flexible hours, including weekends and evenings, as required by the job.

**Requirements**:

- Certificate III or IV in Disability, Aged Care, or Community Services
- Relevant experience working in disability support services
- Excellent communication and interpersonal skills
- Ability to work effectively in a team and independently
- Good problem-solving skills and attention to detail
- Current First Aid Certificate and CPR Certificate
- Current Australian driver's license and access to a reliable vehicle
- Ability to pass a National Police Check and Working with Children Check.

**Salary**: $33.00 - $35.00 per hour

Schedule:

- Monday to Friday
- Public holidays

Supplemental pay types:

- Overtime pay

Ability to commute/relocate:

- Hoppers Crossing, VIC: Reliably commute or planning to relocate before starting work (required)

Work Authorisation:

- Australia (preferred)
