**About us**

headspace Early Psychosis (hEP) provide comprehensive early intervention services to young people aged 12 to 25 years experiencing an early episode of psychosis or at risk of developing psychosis. Our committed and compassionate multi-disciplinary team have a rich history of working with young people, their family / carers to help achieve their recovery goals.

Due to internal staff movements, we currently have several full-time vacancies in our team - two ongoing and one parental leave contract opportunity until October 2025 (with possible extension). There is flexibility to work between 8am and 6pm in all roles.

**About the role**

The positions are within our headspace Early Psychosis Continuing Care Team (CCT) based at headspace Southport. As a CCT Clinician you will provide intensive case management and care coordination to a caseload of up to 20 young people. The role includes assertive outreach and travel within the Gold Coast catchment area to support the engagement of young people within the service. Fleet cars are provided to provide outreach activities.

As a Case Manager, you will be responsible for managing a diverse caseload with varying and complex needs. Your day-to-day role will include assessing and addressing a range of risks, including but not limited to, managing suicidal ideation, monitoring symptoms of psychosis, supporting functional recovery, and providing tailored interventions to support young people in achieving their recovery goals. Additionally, you will play a vital role in linking young people and families to community resources and support networks.

You will be part of a multidisciplinary team (MDT), including psychiatrists fostering a collaborative approach to decision-making. Robust discussions will be encouraged to ensure the best outcomes for young people. You will actively contribute to team development, share your expertise, and support a culture of ongoing learning and growth.

**Who we are looking for**

You are an engaging and compassionate individual with the ability to connect with clients who are navigating mental health challenges and difficult life experiences. You can work both autonomously and collaboratively within a team and contribute a positive team culture and environment.

**Additionally, you will also possess the following skills and experience**:

- Bachelor’s degree in Social Work, Psychology, Occupational Therapy, Counselling or Mental Health Nursing or other relevant field
- An established career in mental health and / or working directly with young people and families. Prior experience in case management or similar transferable skills highly desirable.
- Proven experience (a minimum of 2-3 years) in the mental health field or relevant experience working within the youth sector, demonstrating a strong understanding of the challenges facing young people.
- Proven ability developing case formulation and treatment planning including delivering evidenced based interventions.
- Highly developed written and verbal communication and interpersonal skills, and the ability to engage with young people, advocate on their behalf and establish and maintain effective relationships with staff, clients, carers, and stakeholders.
- Strong time management skills and ability to manage competing demands, while maintaining self-care.
- Ability to use relevant technology and electronic client recording systems and data management tools.
- Passionate about working with young people and committed to making a positive impact in their lives.Eligibility for membership with the relevant professional body such as AHPRA or the Australian Association of Social Workers is highly desirable.

The remuneration for this position will be commensurate with the successful applicant's qualifications, skills and experience and will range between gross $85,120 to $97,377 per annum plus superannuation and salary packaging benefits. You will be required to hold a current Australian Driver’s License and Queensland Working with Children Check.
