Join Dhelkaya Health as our Home Care Packages Coordinator, where you will lead the delivery of our consumer-directed home care packages program. In this dynamic role, you'll oversee care planning, financial management, and service coordination, all while fostering a culture of collaboration and excellence.

You are an experienced leader in aged care or community services with a proven ability to coordinate programs that deliver exceptional, person-centered outcomes. Your expertise includes care planning, financial management, and team leadership, supported by strong communication and organizational skills. You bring a proactive approach to innovation and quality improvement, a commitment to inclusion and diversity, and the ability to build meaningful relationships with clients, carers, and stakeholders.
