**_The Role _**

We are currently looking for compassionate, dedicated and reliable people who take pride in their work.As a support worker you will need excellent communication and listening skills.

**_Main Duties and Responsibilities _**
- Provide 1:1 direct care assistance to each client in accordance with their care plan and as directed.
- Monitor the safety and well-being of the clients and report any concerns to supervisor.
- Support clients to access activities and outings in the community.
- Manage accurate and detailed shift reports, incident reports and progress notes.
- Work with client, family/carer, relevant case manager and other professional to support the client’s individual goals.
- Actively contribute to a team environment through open communication and participation in regular staff meetings.
- Participate in performance development meeting to identifying ongoing training and support opportunities.

**_To be considered for this role you must provide : _**
- Previous experience in supporting people with disabilities
- 100 points of identification ( drivers licence, passport etc)
- National Police Check
- Certificate of completion of NDIS workers orientation program
- COVID-19 infection control training (note, only the infection control module)
- Working with Children's Check (Employee)
- NDIS Worker Screening Check Clearance
- Current First Aid & CPR Certificate **Current Driver’s License & your own car**:

- A Certificate III or IV in one of the following or equivalent: Individual Support/ Home & Community Care/ Aged Care/ Disability/ Nursing
- COVID-19 vaccination ( required )
