Ndis Support Coordinator

**Job Overview**:
As an NDIS Support Coordinator with Innovative Community Care, you will play a pivotal role in assisting individuals with disabilities to navigate the National Disability Insurance Scheme (NDIS). Your primary responsibility will be to collaborate with participants, their families, and service providers to develop, implement, and monitor NDIS plans that best meet the individual's needs and goals. You will serve as a trusted advocate and guide, ensuring that participants receive the appropriate services and supports to enhance their quality of life.
**Key Responsibilities**:
- Work closely with NDIS participants to understand their goals, aspirations, and needs.
- Develop and implement individualised NDIS plans, ensuring alignment with participants' goals and the NDIS guidelines.
- Coordinate and liaise with various service providers, therapists, and community resources to ensure seamless service delivery.
- Assist participants in choosing the most suitable service providers and support options.
- Monitor the progress of participants' NDIS plans and make necessary adjustments to ensure their ongoing effectiveness.
- Maintain accurate and up-to-date documentation, reports, and records related to NDIS plans and participant interactions.
- Provide ongoing support, guidance, and advocacy for participants in their NDIS journey.
- Stay informed about changes in the NDIS policies and regulations and communicate these changes to participants and their families.
**About You**:

**Qualifications/Experience**:
- Minimum Certificate III in Disability or Mental Health/related qualification relevant to the field of human services
- Knowledge and experience using ShiftCare, highly desirable
- Must have, or be willing to obtain a NDIS Worker Screening Check
- Must have, or be willing to obtain a Working with children Check
- Must have, or be willing to complete the NDIS Worker Orientation Module 'Quality, Safety and You'
- Current DHS Working With Children check
- Current driver's license and access to a car
- Knowledge of the NDIS, highly desirable
- Demonstrated experience with NDIS Support Coordination
- Experience with psychosocial disabilities, highly desirable

**Salary**: $59,282.71 - $85,000.00 per year
