Due to steady growth of NDIS clients, we are seeking an NDIS Coordinator to join our team. You will be a part of a small supportive team of experienced Care Coordinators responsible for ensuring that our participants receive suitable high quality and timely services that encourage independence and quality of life.
After-Care has spent the last 25 years building a business centred around supporting people to continue living safely in their own home in our local community. It's why our focus is providing people with the widest possible choice of in-home supports that are person-centred, efficient, easy to access and provide maximum value.
After-Care delivers NDIS supports and Home Care Packages to people living in the suburbs of South East Melbourne and the Mornington Peninsula. We specialise in supporting people who live in the local community and require ongoing personal care supports.
Our team is built on employees at all levels of the company having first hand experience providing care to frail older people and people with disabilities. We value professionalism, equally as much as a passion for caring and finding practical ways to help others.
**What's involved?**
This position is an office-based role that is responsible for:
- Initial rostering and scheduling of care workers to provide cleaning, personal care, community access and respite services in participants homes and the community
- Receiving requests from participants/representatives and helping to clarify/problem solve what supports they require before matching them to suitable care workers
- Maintaining up to date documentation for all participants
- Providing support and supervision to our team of care workers
- Maintaining timely and accurate electronic records using MS Office programs and our CRM Software
**Skills, Qualifications and Experience**
- Demonstrated experience in a client focused position with a commitment to providing superior customer service is essential
- Administrative experience, preferably involving rostering and scheduling of work
- Well established written and verbal communication skills
- Demonstrated ability to prioritise work, manage time efficiently and problem solve
- Computer literacy (Microsoft Office) and familiarity with client information software
- Ability to assess client needs and problem solve issues
- Previous experience (professional or personal) in aged care or disability is highly desirable
- Enrolled Nurse, Cert III or IV in Disability, Individual Support or an equivalent qualification is highly desirable
Pay: $70,000.00 - $85,000.00 per year
Work Authorisation:
- Australia (preferred)
Willingness to travel:
- 50% (preferred)
